What is Compression Therapy?

Compression therapy involves applying pressure to the lower limbs to improve blood flow and reduce swelling, aiding in the healing of wounds. This is achieved using elastic bandages, stockings, or special pumps. The pressure helps move blood back toward the heart, improves oxygenation to the affected area, and reduces fluid buildup. Typically, the highest pressure is applied at the ankle, gradually decreasing up the leg.

This therapy is commonly used to treat venous ulcers and other conditions related to poor blood circulation. Proper application by trained professionals ensures effectiveness and safety.

Benefits of Compression Therapy in Wound Healing

Compression therapy is often used in wound care, especially for venous ulcers. It complements other treatments and enhances the healing process. Here’s how it helps:

Increases Venous Return: Compression applies sustained pressure to the lower limb, helping the calf muscles pump blood more effectively. This boosts venous return to the heart, reducing swelling and minimizing pain.

Improves Microcirculation: By enhancing lymphatic drainage, compression reduces fluid leakage and improves local blood flow and oxygenation at the wound site.

Releases Vasoactive Mediators: Compression therapy aids in releasing anti-inflammatory mediators from endothelial cells. It also decreases peripheral congestion and supports tissue remodeling, crucial for preventing ulcer recurrence.

Your provider will work with you to develop a customized treatment plan that suits your specific wound needs. Compression therapy may be just one part of a comprehensive approach to your care.

 

 

Types of Compression Materials and Use

Various materials and bandages are used that apply different degrees of pressure to the lower limbs. Your provider will work with you to choose the best compression therapy option based on factors such as the degree of pain, the condition of your wound, and its size. Here are some common types of compression materials used in wound care:

Compression Hosiery: Provides graded pressure, highest at the ankle, and comes in various sizes. Knee-high versions are usually sufficient and should be worn over a wound dressing. Replace every six months due to loss of elasticity.

Compression Bandages:

  • Inelastic Bandages: Stiff bandages that provide high pressure when you move but no pressure when resting, ideal for improving circulation and healing.
  • Elastic Bandages: Offer sustained pressure during movement and rest but are less commonly used due to lower pressure.

Multi-Layered Compression Bandages: Consist of four layers and provide adequate pressure without needing extensive training for application.

Intermittent Compression Therapy: Uses a pump to apply and release pressure around the leg, suitable for those who can’t tolerate sustained pressure. Not recommended for patients with heart failure or peripheral arterial disease.

Compression Garments: For patients with lymphedema or phlebo-lymphedema who cannot pull up compression stockings, garments like Farrow or CircAid are useful. These garments provide effective compression and are easier to apply, making them a good alternative for some patients.
